Notice: This document contains template language modeled on industry-standard healthcare disclosures. Before TheraVite Health relies on this language for legal or regulatory compliance — and before this site collects any patient health information — qualified healthcare counsel must review and finalize each policy. Patients and providers should not rely on this document until that review is complete.

This Privacy Policy describes how TheraVite Health, Inc. (“TheraVite,” “we,” “us,” “our”) collects, uses, and protects information you share when you visit theravitehealth.com or use our online services. It does not cover the use of protected health information (PHI), which is governed by our separate Notice of Privacy Practices.

Information we collect

We collect two types of information from website visitors:

  • Information you provide — when you submit a contact form, request information, or download a referral form, we may receive the information you provide (name, email, phone, message content).
  • Information collected automatically — when you visit our site, we collect technical information such as your IP address, browser type, device type, pages visited, and time spent on each page. We do not currently use third-party advertising tracking pixels on patient-facing pages.

How we use information

  • To respond to your inquiries and process referrals or appointment requests
  • To improve our website and services
  • To send you information about our services if you have explicitly requested it (you may unsubscribe at any time)
  • To comply with legal and regulatory requirements

Cookies

Our website may use functional cookies necessary for site operation. We do not currently use marketing or advertising cookies. You can manage cookies through your browser settings.

How we protect information

We use industry-standard security measures including HTTPS encryption for all data in transit and access controls limiting who can view submitted information. For information collected through forms that may include health information, we use HIPAA-compliant processing services with executed Business Associate Agreements.

Your rights

You have the right to know what personal information we hold about you, request a copy, request corrections, or request deletion (subject to legal record-retention requirements for health-related communications). To exercise these rights, contact us at info@theravitehealth.com.

Children’s privacy

Our website is not directed at children under 13, and we do not knowingly collect personal information from children under 13.

Changes to this policy

We may update this Privacy Policy periodically. The “last updated” date at the top of the page indicates the most recent revision.
