TheraVite Health, Inc. is committed to protecting the privacy of your health information. We may use and disclose your health information to provide, coordinate, or manage your care with other healthcare providers involved in your treatment. For example, we may share information with your prescribing physician to coordinate your therapy.
For payment
We may use and disclose your health information to bill and collect payment for the services we provide. For example, we share information with your insurance company to obtain prior authorization or process claims.
For healthcare operations
We may use and disclose your health information to support our business operations: quality assessment, staff training, accreditation activities, business management, and similar activities.
Other uses requiring your authorization
Most uses of psychotherapy notes, marketing communications, and sale of PHI require your specific written authorization. We will not use your information for marketing purposes without your explicit consent.
Other permitted disclosures
We may disclose your health information without your authorization in limited circumstances, including: as required by law, for public health activities, to report abuse or neglect, for health oversight activities, in judicial proceedings, to law enforcement when legally required, to coroners or funeral directors, for organ donation, for approved research, to avert serious threat to health or safety, for specialized government functions, and for workers’ compensation.
Your rights regarding your health information
- Right to inspect and copy — you may inspect and obtain a copy of health information we maintain about you
- Right to request amendment — if you believe information we have is incorrect or incomplete, you may request an amendment
- Right to an accounting of disclosures — you may request a list of certain disclosures we have made of your information
- Right to request restrictions — you may request restrictions on certain uses and disclosures
- Right to request confidential communications — you may request that we communicate with you in a specific way or at a specific location
- Right to a paper copy of this notice — you may request a paper copy of this notice at any time
- Right to be notified of a breach — we will notify you if there is a breach of your unsecured health information
